Carpal Tunnel Syndrome: Pinched Median Nerve at the Wrist

Carpal tunnel syndrome.is by far the most common and widely known of the “pinched nerve” conditions. This article addresses: What is it? Who is at risk for this condition? How is it diagnosed? What kinds of treatments work best?

Carpal tunnel syndrome refers to symptoms caused by entrapment of the median nerve in the carpal tunnel. “Carpal” itself means “wrist,” so a carpal tunnel is nothing more than a wrist tunnel. This particular tunnel can be a crowded place, as it contains not just the median nerve, but nine tendons as well. The “syndrome” consists of some combination of pain, numbness and weakness.

Pain, numbness, or both, are the usual earliest symptoms of carpal tunnel syndrome. Pain can affect the fingers, hand, wrist and forearm, but not usually the upper arm or shoulder. Numbness affects the palm side of the thumb and fingers, but usually spares the little finger because it’s connected to a different nerve.

When weakness is present, it usually indicates that the condition is already severe, and when muscles atrophy (wither) it means the condition is even worse. The affected muscles are those downstream from where the nerve is pinched, and can include those controlling any of three motions of the thumb. In addition, bending of the first knuckles of the index and middle fingers can be affected, as can straightening of the second knuckles of the same fingers. When muscle atrophy is present, it is most evident in the muscular ball at the base of the thumb.

Carpal tunnel syndrome occurs more frequently in women than in men. People who work with their hands a lot – for example to sew, operate hand-tools or perform assembly-line work – are at increased risk for developing this condition. Various medical conditions can also increase the risk of carpal tunnel syndrome, including injuries, arthritis, diabetes, low levels of thyroid hormone and pregnancy. In the case of pregnancy, carpal tunnel syndrome often appears in the third trimester and resolves after the woman delivers.

Optimum diagnosis of this condition combines the time-honored methods of a doctor’s history-taking and physical examination with tests of nerve function called nerve conduction studies. Nerve conduction studies are exquisitely sensitive in detecting impairment of the median nerve at the wrist, particularly when the median nerve is compared with a nearby healthy nerve in the same patient.

In nerve conduction studies, the nerve on one side of the carpal tunnel is activated by a small shock to the skin. An oscilloscope measures how long it takes for the resulting nerve-impulse to arrive on the other side of the carpal tunnel. When the median nerve is pinched, the nerve-impulse is delayed or blocked. Nerve conduction studies are so sensitive that sometimes they show problems that aren’t even causing symptoms. That’s why nerve conduction studies don’t stand alone in diagnosing carpal tunnel syndrome. The examining physician needs to decide if the results make sense for the particular patient in question.

Nerve conduction studies not only show whether or not the median nerve is impaired at the wrist, but also provide precise data concerning how bad the impairment is. In addition, these studies survey the function of other nerves in the arm and hand. Occasionally, a nerve in an adjacent tunnel (the ulnar nerve in Guyon’s canal) can also be pinched. In other cases, nerve conduction studies show that the problem is not one of single nerve-pinches, but rather a more diffuse pattern of nerve-impairment called polyneuropathy. Of course, sometimes the studies are completely normal and suggest that the symptoms are due to something else.

To treat carpal tunnel syndrome, starting with “conservative” treatment makes sense in most cases, especially when the symptoms are still in the mild-to-moderate range. Conservative treatment usually includes a wrist-splint that holds the wrist in a neutral position. In a study published in 2005 researchers at the University of Michigan investigated the effectiveness of wrist-splinting for carpal tunnel syndrome in workers at a Midwestern auto plant. In a randomized, controlled trial – the gold standard method for judging treatments – about half the workers received customized wrist-splints that they wore at night for six weeks. The remaining workers received education about safe workplace procedures, but no splints. After treatment the workers with splints had less pain than those without, and the difference in outcome was still evident after one year.

Conservative treatment might additionally include use of anti-inflammatory medications like aspirin or naproxen, or even steroid drugs. A more intrusive, though still non-surgical, treatment consists of injecting steroid medication into the carpal tunnel itself. This might benefit selected patients, but in a 2005 randomized, controlled study of patients with mild-to-moderate symptoms, researchers at Mersin University in Turkey showed that patients receiving splints did better than those who received steroid injections.

Surgeons can relieve pressure on a pinched median nerve by cutting a constricting, overlying band of tissue. A 2002 study at Vrije University in Amsterdam compared surgical treatment to six weeks of wrist-splinting. After 18 months 90% of the operated patients had a successful outcome compared with 75% in the splinted group.

In some cases it can be reasonable to try conservative treatments without first confirming the diagnosis with nerve conduction studies. However, in the author’s opinion, this risk-free form of testing should be performed prior to any carpal tunnel surgery. (Full disclosure: The author performs nerve conduction studies!)

So we began our tireless search to see if we might find such a rare and wonderful countercurrent to the tide that is sweeping the Muslim religion ever more beyond the shoreline of what sane and civilized people consider blessed.

Were delighted to tell you that, after an extensive search, we were, in fact, able to locate at least one such exceptional and distinguished soul. Obviously, there may be others out there, but theyre just not being visibly vocal, unless, of course, a cartoon shows up that they decide is offensive.

Where, you may ask, did we find our prize Muslim? Why, in the most appropriate of places at Ground Zero, where he is employed as an engineer, working on rebuilding the area after the murderous and damnable crimes of 911. He invited us to meet him on the job, just so we could confirm with our own eyes that a Muslim would be involved in such a noble and redemptive construction process.

The following recounts our entirely delightful and encouraging interview with him, which, we hope, inspirits all to think the better of Muslims in general and, most especially, to persuade many another Muslim to follow in his savingly enlightened footsteps.

NewsLaugh: According to the questionnaire you were kind enough complete, youre a peace-loving Muslim?

Peace-Loving Muslim: Yes, I am.

NewsLaugh: Good for you. We also understand youre modern-minded.

Peace-Loving Muslim: I am, indeed.

NewsLaugh: Glad you meet you.

Peace-Loving Muslim: Glad to meet you, too.

NewsLaugh: Are you the only peace-loving and modern-minded Muslim or do you think there might be others?

Peace-Loving Muslim: Oh, there are millions of them, I assure you.

NewsLaugh: Really? Why dont we ever hear from them?

Peace-Loving Muslim: Because peace-loving people usually dont make news. They wake up and do things like go to work and feed the kids.

NewsLaugh: Yes, of course. Were familiar with such activities ourselves. But why do Muslim clerics so seldom speak out about all the violent activities perpetrated in the name of Islam?

Peace-Loving Muslim: Some do, but I agree, they should be more out there. But you must understand that, since they are peace-loving, they might never shout as loud as Muslims and I I even hesitate to call the most violent ones by the term Muslims who arent. If youll notice, crazy people generally scream, while peaceful people are more like to carry on a conversation, like this one.

NewsLaugh: Yes, of course. But do you think some of them quietly suspect that the monsters perpetrating these crimes are on the side of Allah?

Peace-Loving Muslim: I hope not. I cant imagine a truly religious person seeing Allah as being on the side of murder. But, of course, you have to allow for all possibilities. My own Muslim cleric is a very peaceful fellow who thinks that Osama and his cronies are madmen who are damaging our religion inexcusably.

NewsLaugh: Bless his insightful butt.

Peace-Loving Muslim: Ill let him know you said that.

NewsLaugh: Where were you born?

Peace-Loving Muslim: Pakistan.

NewsLaugh: Oh, where Osama Bin Laden is supposedly hiding out.

Peace-Loving Muslim: I prefer to think of it as the country where President Mushararf is doing everything he can to help the US catch him.

NewsLaugh: Good point. Youre an engineer?

Peace-Loving Muslim: Yes, I am.

NewsLaugh: I suppose that means youve read a number of books besides the Koran. How did you manage to pull that splendid achievement off?

Peace-Loving Muslim: Well, I felt Allah gave me a brain, so He wanted me to use it. I was always good at math. Theres not a lot of math in the Koran. So I guess it didnt interest Mohammad much. But even an elemental look at nature reveals that Allah works with math in ways that are so sophisticated everything happens in perfectly natural ways. The math never gets in the way; it accommodates all events, like the perfectly natural movement of waves. We should only know as much about math as Allah.

NewsLaugh: Well, to tell you the truth, theres not that much math in any religious book I can think of. But thats another interview. How did you become peace-loving? Reading the papers and watching the news, one gets the idea that Muslims, in general, are all a pretty violent and backward lot.

Peace-Loving Muslim: A lot are, I admit it. But thats because a lot of them have erroneous ideas. I cant say they have erroneous knowledge, because I don’t believe there is such a thing as erroneous knowledge. I also don’t believe a lot of knowledge is necessary to be peace loving. My parents, who have very little education, are very peaceful people. So its not a matter of education. Its a matter of outlook. Either you think you serve Allah by killing other people or by loving them. I prefer the latter, and I think Allah prefers my conduct for that and, if He has prepared a paradise for us, peace-loving behavior is the most likely way to get there.

NewsLaugh: Well, it seems like a more logical approach, expecially since Allah or God by any other name made us all. How did you become modern minded.

Peace-Loving Muslim: Well, it was pretty easy. I grew up with my eyes and ears open. While the world around me was often backward, I knew about the world outside. I decided if its modern, thats part of the potential Allah put in it, so I ought to adjust to it and, in fact, do my own best to help move it along. I think the modern world has blessed life with such inarguable enhancements as antibiotics and air conditioning.

NewsLaugh: What do you think the chances are that other Muslims will start to think like you?

Peace-Loving Muslim: Well, it depends on what they hear the loudest; the outside world, ranting at each other, or their inner voice telling them that theyre part of the whole, and, if they want to find a welcome place in the whole, they have to become a peaceful part of it. I think they also have to understand that Muslims running around blowing other people up are acting as if Muslims dont have lives or property that can also be blown up. This is one reason why their actions are harmful. Every time a Muslim blows something or somebody up, we wonder how long it will be before non-Muslims decide to teach us that such behavior is not a one-way street.

NewsLaugh: Yes, we wonder about that, too, It seems to us that the world has been pretty tolerant so far.

Peace-Loving Muslim: To paraphrase you, bless their butts. Yet I cant tell you how much fear peace-loving Muslims live in that the behavior of violent Muslims will result in damage to the nonviolent ones and their property.

NewsLaugh: Well, its certainly a pleasure to meet you. Were glad to know that there is at least one peace-loving and modern-minded Muslim in the world. It gives us hope. May your way of thinking spread.

Peace-Loving Muslim: If it doesnt, I may decide to become a Buddhist. Ive been reading about their beliefs, and, if you have to believe in an ancient religion to make peace with life, it seems like a pretty good option.

Newslaugh: Well, it certainly is a more peaceful choice. Wed dont hear of many Buddhists blowing up non-Buddhists. Whatever you decide, we wish you luck.

Chicago Bears 2006 Preview

The Chicago Bears had a great season in 2005. They finished 11-6 and went to the playoffs. Their one problem was scoring. Many in the media have touted the Bears as the NFC North champions. Are they correct?? Lets take a look at the 2006 Chicago Bears. Fortunately for them, their division is not that tough.

Key Moves

One major move by the Bears was the addition of Brian Griese. The offense was up and down last season. Griese will be fighting for a job. He comes in as a backup, but if Grossman fails to impress, Griese could take the reins. Their defense loses a good player in Jerry Azumah. Despite being only 28, he had to call it quits. His body got the best of him.

Quarterbacks

One of the issues last season was the health of Rex Grossman. Hes expected to be healthy this season. Grossman could help the Bears offense soar. The signing of Brian Griese gives the Bears a solid backup. Even is Grossman goes down, they wont experience a horrendous drop. Neither quarterback is upper echelon. The Bears QB situation is just above average.

Running Backs

The Bears are sitting pretty in the running game. Thomas Jones should have another great year. They also have Cedric Benson in the wings. The running game of the Bears is the offenses strong point. Throw in the tough blocking Bryan Johnson and you have a ground attack that will eat up yards. Thomas Jones had 1,335 yards last season. As nice as that is, Cedric Benson is expected to be better when hes ready.

Receivers

The receiving core is a bit lackluster. Sure, theres Muhsin Muhammad, but hes not getting any younger. Aside from Muhammad, theres not too much to be excited aboutyet. The young Bears receivers do have talent and could blossom into solid contributors on the field. Right now, the receiving core is just average.

Defense and Special Teams

Last season it was all about the defense. The Bears defense constantly made critical plays and kept their team in the game. The 2006 Bears defense is above average again. The best component of the defense is the linebacker unit. They are off the charts. The weak part of the defense is the secondary. Losing Jerry Azumah didnt help any. The Bears did pick up cornerbacks Ricky Manning and Dante Wesley. The secondary will have to step it up in 2006. The special teams unit has its ups and downs. Punter Brad Maynard needs to have a better season. The Bears hope that Robbie Gould, kicker, was not just a one year wonder. On the plus side, the Bears did add a couple return men that could be home run type of guys.

2006 Prediction

Its possible that the Bears win their division, because the division is weak. However, I believe they will be an 8-8 team. The schedule is not too tough, but its not the walk in the park it was in 2005. In 2005, the Bears had a 2-4 record against really good teams. This is why they faltered in the playoffs. They were not as good as their record indicated. In 2006, they have 8 games that look to be pretty tough, 2 that are middle ground and 6 that are against lower tier teams.

We’re in a good mood as long as we get up on time, the kids cooperate and get to school on time, the car starts without a problem, we don’t encounter any traffic jams on the way to work, the boss doesn’t yell, our workload is manageable, etc. This system can work fine as long as everything runs smoothly in our lives.

But that’s not always the case, is it? Situations don’t always adhere to our expectations. Accidents happen. Delays happen. Spouses and bosses can be difficult to please. Vehicles break down. Kids may resist our efforts to keep the schedule running smoothly. What happens to our attitude then? If we don’t make the effort to consciously choose a positive attitude, we run the risk of developing a negative one at random.

How would our lives change if we consciously chose a positive attitude each day? For starters, we’d feel much happier about our lives. We’d feel more motivated about pursuing our dreams because we’d believe in the possibilities for making them a reality. We’d be able to enjoy the time spent with our friends and families more, because we’d want to share our joy with others. We’d feel strong and empowered in our ability to do anything we set our minds to. We’d be able to overcome challenges with ease because we’d have a strong belief in our own skills and abilities. We’d be able to love deeply and completely without fear of rejection or betrayal. We would pause to enjoy the smaller, more precious moments in life, without feeling pressured to do more, be more, or prove our worthiness.

Wow, it sounds awesome, doesn’t it? Is it really possible to feel that way all the time? Yes, but probably not in the way you’re imagining it. Just because we choose a positive attitude doesn’t mean that our lives will transform into smoothly-flowing works of art and stay that way forever. Choosing a positive attitude is a process. We don’t just choose it once and expect everything to be perfect from that moment on. We will still experience setbacks, delays, accidents, frustrations, arguments, fatigue, and fear. The secret is how we choose to react to these experiences. Do we want to let them ruin our day, or do we choose to pick ourselves up and look hopefully to the future again? We do have the power to choose.

You may be asking what the point of a positive attitude is. If we’ll still experience negative circumstances, why bother with choosing a positive attitude? Ah, that’s the true beauty of the creative process. What we choose to focus on the most, we create in our lives.

By choosing a positive attitude each day, we are actually attracting more positive experiences, and reducing the likelihood of negative experiences. You may have noticed that each experience often determines the quality of the next experience, causing a chain reaction in our lives. If one little thing goes wrong, it can throw off our plans for the rest of the day. In this context, it’s easy to see how a positive attitude would be powerful. Rather than allowing one little thing to ruin our day, we would be able to shrug it off and continue on without a hitch.

Over time, this resiliency begins to strengthen and empower us, which will show through in our demeanor. We begin attracting people and experiences that more closely match our attitude, and our lives eventually transform into more positive, fulfilling expressions of joy.

And it all begins with a little thing called attitude! Again, this is a process, so go easy on yourself if you’re trying to adopt a more positive attitude. Simply focus on developing a stronger awareness of your attitude moment to moment in your daily experiences, and begin choosing a more positive one more often. It takes time to realize the full benefits of this type of mindset, but it is time well-spent.

Eskimos are also known to have used Walrus skin to toss each other up into the air before the modern trampoline was born.

George Nissen, a gymnast and competitive diver, and a friend, Larry Griswold designed and built the first modern trampoline in 1934. Inspired by circus trapeze artists, they manufactured the first prototype – a stretched piece of canvas, with grommets inserted along each side, to an angle iron frame by means of coiled springs. It was a great success and trampoline became a household word soon after.

The trampoline has now been used not only for recreational or competitive purposes but also for training in variable body positions in flight to pilots and astronauts.
